Informed Choices provides non-medical, emotional, spiritual, and practical support for individuals and families experiencing end-of-life transitions. We are not a licensed medical provider, counselor, or legal advisor, and our services do not replace professional medical care, mental health therapy, legal advice, or hospice care.

Any guidance provided through this Website or during consultations is based on training and experience as a Certified End of Life Doula. We encourage clients to consult appropriate licensed professionals for medical, legal, or psychological needs.

Informed Choices does not employ licensed physicians, psychologists, or medical practitioners of any kind and that any services performed should not be considered a replacement for the advice and/or services, of a psychiatrist, psychologist, psychotherapist, or doctor.

This company do not represent the sold services as any form of health care or psychotherapy, and by law, makes no health benefit claims for my services. Using the techniques practiced by this company, whether in person, online, in a group, via recording or via proxy, is not a substitute for medical care, advice or treatment. It is not intended for providing psychological counseling. Information given by this company is not intended to create a physician-patient relationship, nor should it be considered a replacement for consultation with a healthcare professional.

Informed Choices and employees do not diagnose or recommend treatment and makes no claims as to healing or recovery from any illness, mental disorders or imbalances. This company does not make recommendations for medication or removal of medications. Please see your doctor for all medical advice.

Guarantee

Although the services offered by Informed Choices have an incredibly high success rate, results are not guaranteed since your own personal success depends on many factors that we have no control over, including willingness and desire to affect the changes within of yourself.

Confidentiality

The Company may discuss aspects of your case with other colleagues keeping your full name and identity completely confidential, unless written permission otherwise is.

Cancellation & Refunds

Due to the nature of the services provided and the need to commit to the practices and teachings provided by Informed Choices to see results, any service offered by this company is non-refundable - unless otherwise noted within a specific product when purchased.
However, the service can be gifted to another person if it has not been fully used.

Refunds & Cancellation

(a) This Agreement may also be terminated by either Party in writing via e-mail.

Written notice shall be provided at least thirty (30) days before the date of intended termination. Upon cancellation by Client, access to the Program and Services will be terminated.

(b) In the event that Client engages in abusive or unprofessional behavior in the Program,

towards representatives of Company or other Program members, if applicable, Company reserves the right to cancel Client’s participation and terminate access to the Services, without notice. No refund will be provided in the event that this takes place. Client will remain obligated to pay all remaining unpaid program fees in full.

(c) Client’s failure to effectively participate in the Program is not grounds for a refund.

(d) Rescheduling.

Sessions/calls will be scheduled using an online scheduling tool. If Client cannot attend a scheduled call, Client shall inform Company with 24 hours’ notice. One courtesy rescheduling will be provided for the term of the Agreement. Any further cancellations of calls will be rescheduled at Company’s discretion, however, will otherwise be considered forfeited by Client. Client is not entitled to a partial or full refund if this forfeiture takes place.

(e) Refund Policy.

Due to the nature of the Services, no refunds can be provided. Client understands that disputing a charge through his or her financial institution (in the form of a “chargeback”) is a violation of this Agreement and agrees to not do so. Please refer to Section 2(c) for our chargeback policy.

(f) The Program and Term cannot be paused or placed on hold for any reason while program is in session (once an automated payment or one time payment is received, the program is considered in session) without the written authorization of Company.

(g) Force Majeure. Notwithstanding the above, the Company may choose to be excused of any further performance obligations in the event of a disastrous occurrence outside the control of Company that materially affects the Services provided in this Agreement, including:

1. A natural disaster (fires, explosions, earthquakes, hurricane, flooding, storms, or infestation); or

2. War, invasion, act of foreign enemies, embargo, or other hostility (whether declared or not); or

3. Sickness of provider or provider immediate family; or

4. Any hazardous situation created outside the control of either party such as a riot, disorder, pandemic or epidemic, nuclear leak or explosion, or act or threat of terrorism. In the event that Section 5(g) applies, Company will be permitted to make a reasonable effort to reschedule calls/sessions/etc. as needed in order to comply with the terms of this Agreement, however, will not be found in breach if this is not possible due to the circumstances.

Intellectual Property

All content on this Website, including but not limited to text, graphics, logos, images, and downloadable materials, is the property of Informed Choices or its content suppliers and is protected by copyright and other intellectual property laws.

You may not reproduce, republish, copy, upload, post, transmit, or distribute any material from this Website without our prior written permission.

Limitations

In no event shall Company or its suppliers be liable for any damages (including, without limitation, damages for loss of data or profit, or due to business interruption,) arising out of the use or inability to use the materials on Companies Internet site, even if Company or a Company authorized representative has been notified orally or in writing of the possibility of such damage. Because some jurisdictions do not allow limitations on implied warranties, or limitations of liability for consequential or incidental damages, these limitations may not apply to you.

As an Instructor, You represent, warrant, and covenant that: No Submitted Content shall infringe or misappropriate any intellectual property right of a third party. But, if instructor use HEVC compression, it's the instructor's responsibility to pay the royalties to the MPEG-LA, the owner of HEVC patents as appropriate.
